Job Description

Location: Based in Campbelltown on the lands of the Dharawal People
Employment Type: Part-time, 2 days a week until June 2025
Hourly Rate: Up to $43.00 per hour

The Position

As a Social Worker in the My Care Partners program, you will focus on improving the health and well-being of individuals, families, and communities by addressing social determinants of health and providing essential support services. You will play a vital role in enhancing community resilience, improving health outcomes, and reducing health disparities with at risk individuals. Strong communication skills, cultural competence, and the ability to work collaboratively are essential for success in this role.

What You’ll be Doing

  • Conduct thorough psychosocial and risk assessments to develop patient goals and access community support.
  • Provide counselling for issues like depression, anxiety, grief, and family dynamics for patients enrolled in the program.
  • Collaborate closely with Registered Nurse Care Coordinators to address psychosocial issues impacting patient health.
  • Develop care plans, support patient self-management, and make referrals to appropriate services.
  • Foster inclusive care for diverse communities, including LGBTIQA+, CALD, and Aboriginal and Torres Strait Islander individuals.
  • Participate in clinical meetings and case reviews to ensure cohesive, team-based care delivery.

What You’ll Bring

  • Qualifications: Bachelor’s Degree in Social Work, with eligibility for membership in the Australian Association of Social Workers (AASW).
  • Experience: Minimum of 2 years working as a registered Social Worker, with a focus on mental health care, crisis intervention, and short-term treatments.
  • Skills: Advanced clinical skills in managing complex cases, excellent communication abilities, and a commitment to inclusive, patient-centred care.
  • Diversity Commitment: Experience working with diverse communities and addressing unique needs in mental health and chronic disease.
  • License: Current driver’s license.
