Program Officer, Aged Care Capability Building

  • Utilise your project management skills to build the capability of Residential Aged Care Homes (RACHs) throughout the north west Melbourne region
  • Full Time (1.0 FTE), Maximum Term role until 19 May 2025 (Parental Leave cover)
  • Base remuneration starting from approx $70K + superannuation + salary packaging
  • Generous NFP salary packaging options up to $18,550
  • Docklands office location with option to work some days at home

The Program Officer, Aged Care Capability Building will be responsible for engaging with and providing support to Residential Aged Care Homes (RACHs) in the north western Melbourne region. This will involve providing guidance to assist participating RACHs to develop and implement a variety of programs e.g. The Telehealth training package and General Practice Aged Care Incentive. The Program Officer will also actively collaborate with other PHN staff and teams on aged care and older adults’ initiatives with a focus on improving the health outcomes of older adults in the NWMPHN region.

What are PHNs?

PHNs have been established with two key objectives:

  • increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Engage with health care providers including, but not limited to, residential aged care providers and staff, pharmacists, allied health professionals and local hospital networks
  • Support the implementation of enabling tools, resources and guides to build the capability of health care providers. This could include education, training and digital platforms.
  • Ensure capability building activities are delivered efficiently and effectively to drive best practice and enabling a consistently high standard across the organization
  • Record and share feedback, challenges and enablers from health care providers
  • Ensure projects are delivered in accordance with funding guidelines, contract requirements and organisational procurement policies.

We are looking for someone who has:

  • Qualification and/or experience in health, project management, aged care, health promotion or similar
  • Project management – experience in scoping, planning, monitoring, and reporting on projects
  • Managing work in a complex environment, with excellent organisational skills
  • Communicating complex information to a range of audiences
  • Building effective relationships and working collaboratively
  • Understanding of how to use and interpret data to guide improvement
  • Well-developed written and verbal communication including report writing and presenting
  • Ability and competence in the use of technology and a variety of software systems, including competency with MS Office (Excel, Word, PowerPoint)

The successful candidate will be required to undertake a National Police Check and Victorian Working with Children Check and hold a drivers licence.
