Community Response Officer

This position is for an identified First Nations person.

The successful applicant will have experience working with First Nations community, strong communication skills, be able to work in an effective and timely manner, work independently with good planning and organisational skills, and be a self motivated person with a positive strength-based approach to supporting others. The ability to rely on experience and judgment to plan and accomplish goals and a wide degree of creativity and latitude is expected

As an Indigenous Community Response Officer – Youth and Elders Support Program  is to achieve but not limited to the following outcomes

  • Increased opportunities for early intervention support and education
  • Increased opportunities to engage with and leverage existing community infrastructure to access reputable health and aged care information
  • Increased knowledge, skills and confidence for Elders, carers and families to navigate the health and aged care services
  • Increased confidence for elders to manage their health and well-being
  • Elders to experience a more integrated and connected system to manage their health and aged care supports
  • Connections to culture
  • Engagement in the education setting

Key responsibilities within this role include but are not limited to:

  • Co-design culturally specific youth and health ageing activities to support the needs of the local community
  • Deliver activities to engage youth and older people (eg) planned activities, presentations, group sessions, individual support where required, resource development within or outside the community
  • Promote culturally appropriate youth and health ageing activities to the local community
  • Evaluate activities by collecting data and participant feedback
  • Collaborate with the broader health and aged care system to improve outcomes for community members
  • Connect with local subject matter experts and relevant professionals to deliver information sessions and activities
  • Comply with reporting requirements;
  • Ensuring contractual requirements and KPI’s are achieved (please refer to attached Schedule for detail)
  • Liaising with relevant Stakeholders;
  • Assist with preparing reports;
  • Working alongside all Gunya Meta staff to develop relationships in order to target Contractual and Internal KPI’s;
  • Ensure administration tasks are completed in a timely manner.
  • Ensure program participants have completed all relevant paperwork before commencing a program;
