Nurse Care Coordinator (Chronic Disease)

Location: Based in Campbelltown on the lands of the Dharawal People

Employment Type: 2 positions, hours negotiable, maximum term until June 30 2025

Hourly Rate: $51.76 per hour

The Position

As a Chronic Disease Registered Nurse Care Coordinator, you will play a pivotal role in delivering tailored care, helping patients manage their health from the comfort of their homes. You’ll provide expert support in navigating their care, bridging gaps, and promoting self-management strategies that lead to positive health outcomes.

What You’ll be Doing

  • Conduct comprehensive health assessments, providing tailored recommendations for chronic disease management.
  • Deliver care coordination services to at-risk patients, ensuring smooth transitions between care settings.
  • Foster collaborative relationships between general practice, community health services, and acute care providers.
  • Improve patient experiences, quality of life, and reduce preventable hospitalisations through targeted interventions.
  • Champion inclusive care for diverse communities, including LGBTIQA+, CALD, and Aboriginal and Torres Strait Islander people.
  • Engage with patients to set health goals, develop care plans, and support self-advocacy.

What You’ll Bring

  • Qualifications: Bachelor of Applied Science (Nursing), Division 1 Registered Nurse with current AHPRA registration.
  • Experience: Advanced clinical skills in managing chronic diseases and complex care coordination.

  • Communication: Excellent interpersonal skills with the ability to collaborate across a wide range of clinical settings.
